How is it that sometimes you can tell what's going on in someone's mind or maybe a struggle or something like that...even though they never gave a clue, a hint, or said anything to make you think so?

Lots of ways, to be honest... sometimes it's just knowledge of human nature, or the particular practices of a certain person's mannerisms, and sometimes it's just a heaven-sent gift meant to help you help them. and then sometimes, you just can't explain it.
